Whether you keep it to yourself or say it aloud is your business, but with that said, don't worry about telling peopke about it. If your fans truly care, then they'd know and appreciate that your health comes first.

@JimmyBiscuit and if they don't, I believe the best way to handle that is to make a fist, extend your arm and extend your middle finger.
